Anyone in the UK can register with us. You can register in one of three ways:

Download: Print, and complete the registration form here, then email, post, or bring it to us.

In person: Visit reception and complete a registration form.

Home visits are only available for patients who live within the practice boundary* and meet the clinical criteria.

*The practice visiting boundary covers the following areas: LS13 1, LS13 3, LS13 2, LS13 4, LS12 3, LS11 0, LS10 4.
